Welcome to Evolve Aesthetics and Wellness. We are committed to providing high-quality care in regenerative medicine, sports medicine, non-operative orthopedics, wellness, and aesthetic treatments. To ensure a seamless experience, we have outlined our policies and terms below.

1. Appointments and Scheduling- All appointments are scheduled by phone or online through our booking platform. - A valid credit card is required to secure all appointments. - We kindly request that new patients complete all necessary forms before the first appointment to ensure timely service. - Late arrivals may result in a shortened treatment time to accommodate other patients.

2. Cancellation and No-Show Policy- Cancellations or rescheduling must be made at least 24 hours before the scheduled appointment. - A cancellation fee may apply if notice is not given within this timeframe. - No-shows will be charged 50% of the scheduled service fee.

3. Payment and Insurance  - We accept all major credit cards, debit cards, and cash. Payment is due at the time of service. - Our services are not covered by insurance, and we do not bill insurance companies directly. However, we are happy to provide a detailed receipt for you to submit to your insurance for reimbursement if applicable. - Financing options may be available for certain treatments—please inquire for more information.

4. Refunds and Returns  - All services provided are non-refundable. Results from treatments may vary, and no guarantees are made on outcomes. - Products purchased from our office may be returned or exchanged if unopened and in the original condition within 14 days of purchase. Refunds will be processed to the original form of payment.

5. Medical and Aesthetic Treatments- Evolve Aesthetics and Wellness specializes in both medical and non-medical treatments, including PRP, BMAC, microneedling, and cosmetic injectables. - Our practitioners will evaluate your individual health needs before recommending a treatment plan. - Please inform us of any medical conditions, allergies, or medications prior to your appointment. - You must follow pre- and post-treatment instructions carefully to achieve the best results and reduce the risk of complications.

6. Confidentiality and Privacy  - We are committed to protecting your privacy and personal information. All patient records are confidential and maintained in compliance with HIPAA regulations. - Your information will not be shared with any third parties without your consent, except as required by law.

7. Liability Waiver- By choosing to undergo treatment at Evolve Aesthetics and Wellness, you acknowledge that you have been informed of potential risks and side effects. - You agree to release and hold harmless Evolve Aesthetics and Wellness and its staff from any liability related to adverse outcomes or reactions to treatment.

8. Right to Refuse Service- We reserve the right to refuse treatment to any individual if we believe it is not in their best interest or if they do not follow pre-treatment and post-treatment instructions.

9. Communication and Consent- By providing your contact information, you agree to receive communication from us via email, text message, or phone call regarding appointments, treatment reminders, and promotions. - You may opt out of marketing communications at any time.
